Ledger of birth and death records for Eagle Creek Township. The ledger has hard black book cloth covers reinforced with red leather at the corners and spine. The bound paper pages are printed in red, black and blue. Printed on the title page is "Booth's Registry of Births and Deaths for clerks and health officers to register the facts concerning births and deaths occurring in their respective towns and villages" along with maker's marks. The ledger contains a register of births by year (date, name of child, sex, color, name of father, father's place of birth, name of mother, mother's place of birth, date of record) and a register of deaths by year (date, name of deceased, sex, color, marital status, age, place of birth, name of parents, parents place of birth, cause of death, date of record).
