A blank COVID-19 vaccination card distributed by the Centers for Disease Control (CDC) to various local outlets all over the country. This particular card came from Medicap Pharmacy in Austin and has a sticker on it with pre-filled information for the Pfizer vaccine with the lot number and expiration date. As people were vaccinated, their information was filled out onto cards like these so they could easily keep track of vaccinations (and boosters) received. The cards also provided an efficient and easy way for people to prove they were vaccinated during the brief window of time when many employers and/or venues required vaccination for entry.
